Telehealth Weight Management $25
This 30-minute telehealth consultation is designed to help you learn more about our medically supervised weight management program and determine if it is the right fit for your goals. During this visit, you will have the opportunity to ask questions, review your health history, discuss expectations, and explore personalized treatment options.
A $25 consultation fee is required at the time of booking. This fee will be waived or refunded if you choose to enroll in the weight management program.

This extended appointment allows time to review your background and create a personalized care plan.

For established patients (18+) continuing their care at 521 Wellness.
These visits are designed for follow-ups, medication adjustments, and review of chronic conditions or new symptoms.

GLP-1 Injections: Semaglutide & Tirzepatide
At 521 Wellness, we provide a structured, clinically supervised weight loss program using GLP-1 medications such as Semaglutide and Tirzepatide. These medications work by improving satiety, regulating appetite, supporting metabolic health, and helping patients achieve sustainable long-term weight loss.
Your initial consultation may be completed via telehealth or in-person, allowing flexible access to care based on your preference and clinical needs.

Payment & Billing Information
At 521 Wellness, we value transparency and simplicity when it comes to billing and payment.
Payment at Time of Service
All prices listed are cash visit rates, unless services are submitted to insurance for billing.
Payment is due at the time of service. If your visit will be billed to insurance, your copay or coinsurance will be collected at that time.
Insurance Billing & Reimbursement
If your insurance plan is billed, 521 Wellness will submit the claim on your behalf.
A superbill will also be issued along with a claim submission form for patients or families who wish to submit directly to their insurance for potential reimbursement.
Laboratory & Imaging Services
Lab tests ordered during your visit will be billed directly by the laboratory to your insurance provider.
Imaging services (such as X-rays, ultrasounds, or CT scans) will be billed directly by the imaging facility to your insurance provider.

Hello, I’m Diana Bucher, MSN, APRN-CNP, AGPCNP, founder and provider at 521 Wellness.
I’m a certified Adult-Gerontology Primary Care Nurse Practitioner with over a decade of combined nursing experience across skilled nursing, primary care, and community-based settings.
I created 521 Wellness to bring back what healthcare should be — personal, accessible, and focused on genuine relationships. My approach blends preventive care, patient education, and individualized treatment to support each person’s unique health goals.
